Identification – Adult

This template is for you to complete and have with you in case the adult in your care has a significant disability that could result in them getting lost and being unable to identify themselves. This could easily occur when you are busy in a department store and the adult in your care wanders off. It is an immediate reference for you to show store personnel to assist you or the police should they need to be called to assist in locating the adult. Remember to keep the photo current.

It can also be used for those without a significant disability, if you are aware that you have further medical conditions or anxiety that may put you at risk in a stressful situation. It will then serve as a means of identification and will outline your needs.
